TERMS AND CONDITIONS

*My child(ren) has no limitations, injuries, or conditions that would affect said child’s ability to participate safely in this program. In addition, I grant permission for my child's image to appear on the Endeavor Plus or School Facebook/Twitter accounts as well as webpage.

* Cancellations are available given 30 days notice via email. Verbal notice is not a valid form of cancellation. Contact the school with the notice or any questions.

* Students participating must adhere to the rules set forth by the instructors at all times.

PHOTO/VIDEO/SOCIAL MEDIA POLICY

*I agree to allow Endeavor Plus to take photographs and video of my child and to use on its websites and social media pages including Instagram, Twitter, Facebook and promotional materials without compensation. I understand that my child's identity will be kept confidential.

Waiver of Liability:

*In consideration of being permitted to attend Endeavor Plus on behalf of myself, my family, my heirs, and my assigns, I hereby release Endeavor Plus , LLC, its agents, and its employees from liability for injury, loss, or death to the registered participant while using any facility or equipment or in any way associated with participating in the activities Let’s Move-Class offers now or in the future.

*I have read the preceding information and do not need further clarification. I know, understand, and appreciate the risks associated with participation in Endeavor Plus and I am voluntarily enrolling my child in the activity. Endeavor Plus takes every measure to create and maintain the safest environment possible at all times; however, I acknowledge that the participant is assuming all of the inherent risks of the activity. I further understand that in the event of a medical emergency, Endeavor Plus will call EMS to render assistance and that I will be financially responsible for any expenses involved.

I accept these conditions for my child(ren)’s participation in the Endeavor Plus program.
