Endeavor Plus Terms and Conditions

Behavior Policy

·        Students participating must adhere to the rules set forth by the instructors at all times.

Liability Waiver

·        My child(ren) has no limitations, injuries, or conditions that would affect said child’s ability to participate safely in this program.

·        In consideration of being permitted to attend Endeavor Plus Classes (DanceStars, Active Athletes,

·        STEM, ART and others) on behalf of myself, my family, my heirs, and my assigns, I hereby release Endeavor Plus , LLC, its agents, and its employees from liability for injury, loss, or death to the registered participant while using any facility or equipment or in any way associated with participating in the activities Endeavor Plus offers now or in the future.

·        I have read the preceding information and do not need further clarification. I know, understand, and appreciate the risks associated with participation in the activity and I am voluntarily enrolling my child in the activity. Endeavor Plus takes every measure to create and maintain the safest environment possible at all times; however, I acknowledge that the participant is assuming all of the inherent risks of the activity. I further understand that in the event of a medical emergency, Endeavor Plus will call EMS to render assistance and that I will be financially responsible for any expenses involved.

Social Media and Photo Release

·        I grant permission for my child's image to appear on the Endeavor Plus or School Facebook/Twitter accounts as well as webpage.

·        I agree to allow Endeavor Plus to take photographs and video of my child and to use on its

·        websites and social media pages including letsmove.endeavorschools.com, Instagram, Twitter, Facebook and promotional materials without compensation. I understand that my child's identity will be kept confidential.

Refunds

·        Refunds/make-up sessions will not be made for non-attendance for a scheduled session, including if your child chooses not to take part in the session for any reason.

·        Refunds/make-up sessions will not be made for school related absences, such as field trips, photo days, or other similar instances. (Please note, we make every effort to work within the school’s schedule to avoid such incidents as much as possible.)

Cancellations

·        Cancellations are available given 30 days notice via email. Verbal notice is not a valid form of cancellation. Contact the school admin with the notice or any questions.

·        Cancellations are not prorated or refunded. Upon enactment of Cancellation, the auto-rebilling

·        ceases.

Billing

·        Our course fee is set up as 9 installment payments for 32 lessons spread throughout the year. Active Athletes Dance, and Art are 9 payments of $68.50 while Stem is 9 payments of $79.

Class Meetings and Schedule

·        There will be 32 class meetings through the year. Day and time of the week are subject to change. There may be weeks where more than 1 class is conducted and weeks where none are, due to school circumstances. Classes will not be made up if your child is not present or unwilling to participate.
